Following the Province of Ontario and the Township of Woolwich recreation facilities regulations, beginning September 22, 2021, proof of full COVID-19 vaccination will be required for all individuals entering the Woolwich Seniors Association. In cooperation with the Township of Woolwich and to maintain a central proof of vaccination system at the WMC, all Woolwich Seniors Association members are requested to enter through the main Woolwich Memorial Centre doors, facing Snyder Avenue. The outdoor senior’s entrance will not be open.
 
Along with your proof of full vaccination, please bring another form of identification:
• Birth Certificate
• Citizenship Card
• Driver’s Licence
• Government (Ontario or other) issued identification card, including health card
• Indian status card/Indigenous membership card
• Passport
• Permanent resident card
Individuals can print or download their vaccination receipt from the provincial booking portal. If you need assistance in getting a copy of your receipt, please contact the Woolwich Seniors Association office at 519-669-5044 or email contact@woolwichseniors.ca.
Beginning October 22, 2021 individuals can show proof of vaccination with their Government of Ontario-issued QR code on their phones.
COVID-19 Screening will be done on entry to the WMC. Where possible, build in extra time to allow for necessary screening and vaccine verification when coming to the centre. We ask for your cooperation and patience as we continue to adapt to the changing needs of
